CHART u​ses a multi-level approach to improve the overall health of heart failure patients. Both doctors and heart failure patients receive educational information from CHART. We aim to improve doctor recommendations and the heart failure patient’s ability to take care of their condition. There are two treatment strategies being tested, Enhanced Education and Enhanced Training.  A brief description follows below.​​​​

We will provide doctors:

  • The 2010 Heart Failure Guidelines 

We will provide heart failure patients:

  • Six comprehensive educational mailings on managing your heart failure from the American Heart Association

We will provide doctors:

  • The 2010 Heart Failure Guidelines and a po​cket-sized laminated card that summarizes the guidelines
  • A report on their CHART heart failure patient indicating if the patient’s care is in line with evidence-based practices
  • Additional training on heart failure through an online educational course (1 CME credit)


We will provide heart failure patients:

  • Six comprehensive educational mailings on managing your heart failure from the American Heart Association
  • In-home visits and telephone contacts provided by a community health worker for 6 months
  • Monthly telephone contacts provided by a community health worker for 2 years​
