​LIFE is a community-based program located in the inner city of Chicago which helps African American adults manage their type-2 diabetes through diet and exercise. Individuals who are eligible to participate are assigned to one of two groups. One group receives 2 diabetes education classes led by a registered dietitian. The other group receives 22 culturally-tailored self management classes led by a registered dietitian and peer supporters. These classes will be held at your local park district.
You may be eligible to participate in LIFE if you can answer “Yes” to the following questions.
  • 1. Are you 18 years of age or older?
  • 2. Are you African American?
  • ​3. Have you been diagnosed with type-2 diabetes?
  • ​4. Are you on medication for type-2 diabetes?
